1. Introduction

RocketForms (“Service,” “we,” or “us”) is an internal forms management platform operated by HealthRx Ventures Private Limited for use by authorized RocketHealth team members.

These Terms of Use govern your access to and use of RocketForms, including all features, functionalities, and related services. By accessing or using the Service, you acknowledge that you have read, understood, and agree to be bound by these terms.

2. Account Access and Authorization

Access to RocketForms is restricted to authorized RocketHealth team members with valid @rockethealth.com email addresses.

Account Responsibilities

You must:

• Provide accurate and complete information during account setup
• Keep your account credentials confidential
• Immediately notify us of any unauthorized access or security breaches
• Use only your own authorized account (no account sharing)
• Comply with RocketHealth's internal security policies

3. License and Intellectual Property

Unless otherwise stated, RocketHealth and/or its licensors own all intellectual property rights in RocketForms and all related materials, including but not limited to:

• Software code and architecture
• User interface and design
• Documentation and materials
• Trademarks, logos, and branding

Grant of License

RocketHealth grants you a non-exclusive, non-transferable, revocable license to use RocketForms solely for internal business purposes in accordance with these terms.

Prohibited Activities

You may not:

• Reverse engineer, decompile, or attempt to derive source code
• Copy, reproduce, or distribute content for commercial purposes
• Modify or create derivative works without written permission
• Remove or obscure proprietary notices or labels
• Use the Service to build a competitive product

4. Form Creation and Management

Form Creation Guidelines

When creating forms on RocketForms, you must:

• Ensure questions comply with healthcare privacy regulations (HIPAA, local data protection laws)
• Use clear, professional language appropriate for patient interactions
• Configure appropriate webhook integrations for data synchronization
• Select suitable themes that maintain RocketHealth brand consistency
• Set correct regional configurations (India, UAE, UK, USA, Canada, Australia)

Response Handling

You are responsible for:

• Promptly reviewing and acting on form submissions
• Maintaining confidentiality of patient/user responses
• Deleting outdated or unnecessary form data in accordance with retention policies
• Exporting and backing up critical response data as needed

Webhook and Integration Security

When configuring webhooks:

• Use secure HTTPS endpoints only
• Verify HMAC SHA256 signatures on received data
• Keep webhook secrets confidential
• Monitor webhook delivery status and address failures promptly

5. Acceptable Use Policy

You agree not to use RocketForms to:

• Engage in unlawful, illegal, fraudulent, or harmful activities
• Transmit malware, viruses, or malicious code
• Conduct automated data collection (scraping, mining, extraction) without written consent
• Send unsolicited commercial communications
• Impersonate another person or entity
• Interfere with or disrupt the Service or servers
• Violate any applicable laws or regulations
• Collect or harvest personal information without consent

6. Healthcare Service Facilitation

RocketForms is a tool for collecting patient information to facilitate RocketHealth's healthcare services.

7. Content and Data Usage

Information submitted through RocketForms may include:

• User identification data (name, email, phone)
• Form submissions and responses
• Usage analytics and metadata
• File uploads and attachments
• UTM tracking parameters

By submitting content through RocketForms, you grant RocketHealth a non-exclusive, irrevocable, perpetual, worldwide, royalty-free license to use, process, and store this information for:

• Service provision and improvement
• Healthcare delivery and patient care
• Analytics and reporting
• Compliance with legal and regulatory requirements

8. Disclaimers and Limitations of Liability

Limitation of Liability

RocketHealth shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of or inability to use the Service.

9. Indemnification

You agree to indemnify, defend, and hold harmless RocketHealth, its parent companies, subsidiaries, affiliates, officers, directors, employees, and agents from and against any and all losses, damages, costs, liabilities, and expenses (including reasonable attorneys' fees) arising from:

• Your breach of these Terms of Use
• Your violation of any applicable laws or regulations
• Your use of RocketForms in a manner not authorized by these terms
• Claims that content you submitted infringes third-party rights
• Your negligent or willful misconduct

10. Account Termination

Voluntary Termination

You may close your account at any time by emailing [email protected]. Upon termination, your access will be revoked, though form response data may be retained for compliance purposes.

Involuntary Termination

RocketHealth may suspend or terminate your access to RocketForms at any time, with or without notice, for:

• Violation of these Terms of Use
• Unauthorized or fraudulent use of the Service
• Security breaches or misuse of data
• Termination of employment with RocketHealth
• Any other reason deemed necessary by RocketHealth

Effect of Termination

Upon termination:

• Your license to use RocketForms immediately ceases
• You must cease all use of the Service
• Forms created by you may be archived or transferred to another team member
• Provisions regarding liability, indemnification, and dispute resolution survive termination

11. Modification of Terms

RocketHealth may revise these Terms of Use from time to time at its sole discretion. Changes will be posted on this page with an updated “Last updated” date.

Your continued use of RocketForms after modifications constitutes acceptance of the updated terms. We recommend reviewing this page regularly to stay informed of any changes.

12. Dispute Resolution and Arbitration

Any dispute or differences arising from the Services availed herein or any connected issue shall be referred to adjudication by a sole arbitrator mutually appointed by the parties.

• Jurisdiction: Courts of Gaya, Bihar have exclusive jurisdiction
• Language: Proceedings shall be conducted in English
• Costs: Each party shall bear its own costs unless otherwise determined by the arbitrator

RocketForms operates under Indian jurisdiction and complies with:

• Digital Personal Data Protection Act, 2023
• Applicable healthcare and medical data protection regulations
• Other relevant Indian laws and regulations

These Terms of Use shall be governed by and construed in accordance with the laws of India, without regard to conflict of law principles.

14. Miscellaneous Provisions

Entire Agreement

These Terms of Use constitute the complete agreement between you and RocketHealth regarding the use of RocketForms and supersede all prior agreements, understandings, and communications.

Severability

If any provision of these terms is found to be unenforceable or invalid, that provision shall be limited or eliminated to the minimum extent necessary, and the remaining provisions shall remain in full force and effect.

Waiver

No waiver of any term or condition shall be deemed a further or continuing waiver of such term or any other term. RocketHealth's failure to assert any right under these terms shall not constitute a waiver of that right.

Assignment

You may not assign or transfer these terms or your rights hereunder without RocketHealth's prior written consent. RocketHealth may assign these terms without restriction.
